Submitting a Claim
You can download the claim form from our website, or we can fax or mail a copy to you. For a pet's first claim, we need the pet's complete medical records from both current and previous veterinary or emergency clinics. Both you and your veterinarian will need to complete our one page claim form. The form is then faxed or mailed to Petsurance with a copy of your receipt. If the claim form is incomplete it will be returned to you for additional information. We may request a full history from your veterinarian(s). If we request additional information, the approval will be delayed until we have all the requested information. Have questions? Contact us at 866-715-6098.
Adding a Pet
To make sure all your pets are covered with Petsurance, please call us at 866-715-6098 to add additional pets to your existing policy.
Updating Contact Information
To make any updates to your contact or payment information, please call us at 866-715-6098 to assist you.
Adding Additional Coverage
You can currently add coverage for Hip Dysplasia to your policy. To do so, simply call 866-715-6098 and one of our member specialists will be happy to assist.
Canceling a Policy
You may elect to cancel your Petsurance pet insurance policy at any time by giving notice to Petsurance by calling 866-715-6098. You may be required to return the original policy or sign a "policy release," and of course you will be responsible for any premium earned through the date of cancellation.
